Midline partners with Kinetic as its official clinic location and billing manager for patients, so appointment requests are processed through Kinetic’s system.
To schedule a visit with Midline, select 'Marcilet Miller' as your clinician and follow the prompts to complete your booking.
Your appointment will take place in Midline’s dedicated space within Kinetic Physical Therapy.
Kinetic’s newly renovated space provides Midline patients with a well-equipped, private, comfortable, and luxurious environment for their treatment.

Credit card: required to hold your appointment. You will not be charged now.
Insurance & Billing
Midline accepts cash payment for all services and certain Blue Cross Blue Shield (BCBS) plans for physical therapy treatment. Please call your insurance company to verify your benefits and out-of-pocket expenses. Kinetic Physical Therapy will file the claims on your behalf.
Midline is an out-of-network provider for all other insurance companies. We are happy to assist you by providing an itemized receipt, which you can send, as is, to your insurance company for reimbursement.

1 hour physical therapy visit
$175: 1-on-1 in-office visit
$200: 1-on-1 at-home visit
30-min wellness visit
$100: In-office visit, including dry needling + manual therapy
+$35: Cupping addition
